12/08/2011“Applying Advanced FMEA Methods to Vehicle Fire Cause Determinations” / rFMEA, presented by Kerry Parrott with Pat Mattes and Doug Stahl at 2011 ASME IMECE

Fort Wayne, Indiana – December 9, 2011 – Kerry Parrott, BSME CFI CFEI/CVFI of Stahl Engineering & Failure Analysis, LLC recently presented Applying Advanced FMEA Methods to Vehicle Fire Cause Determinations at the 2011 ASME International Mechanical Engineering Congress and Exposition in Denver, Colorado. Pat Mattes, an independent Fort Wayne quality tools consultant, and Douglas R. Stahl PE, also of Stahl Engineering & Failure Analysis, LLC, were co-authors.

This paper introduced the “reverse FMEA” (rFMEA) failure/root cause analysis technique, developed by the authors, by applying advanced Failure Modes and Effects Analysis (FMEA) techniques and methodology to a failure analysis investigation in the “reverse” direction, relative to a traditional predictive FMEA process. An example of this rFMEA technique was demonstrated as applied to a vehicle fire origin and cause failure analysis investigation. The rFMEA analysis technique meets the requirements of NFPA 921 Guide for Fire & Explosion Investigations to apply a systematic approach utilizing the scientific method for fire origin and cause determinations.
08/23/2010Kerry Parrott presented “Commercial Vehicles, How & Why They Burn”
Fort Wayne, Indiana – August 23, 2010 - Kerry Parrott presented “Commercial Vehicles, How & Why They Burn” to the 2010 33rd Annual PAAI (Pennsylvania Association of Arson Investigators) Seminar focused on Vehicle Fire Investigation.  Mr. Parrott, along with several other vehicle fire experts, was invited to present at the Pennsylvania Association of Arson Investigators 2010 33rd Annual Seminar in State College, Pennsylvania, focused this year on vehicle fire investigation.  His presentation provided background information on commercial vehicles and equipment and the special challenges of conducting a proper origin and cause fire investigation and analysis on these type of products.  Additionally, he reviewed several case studies of commercial vehicle fires dealing with specific fire origin determination and cause analysis.
